The accident happened around 6 p.m. Monday along Highway 199 near milepost 25, according to a press release from the Oregon State Police.
Police say a 2004 Subaru Legacy driven by Ruark Miller, 24, of Cave Junction was southbound on the highway.
The Subaru crossed the centerline and crashed head-on with a northbound 1984 Dodge pickup driven by 48-year David Wonderling of Selma.
A fire started in the pickup but was extinguished by people who stopped to help.
Miller was taken to Asante Three Rivers Medical Center and was treated for non-life threatening injuries. Police say he was not using safety restraints.
Wonderling had two passengers in his truck. He and a 12-year-old girl were taken to Three Rivers, where they were treated and released.
The second passenger, 68-year-old Joyce Wilson of Selma, was taken by helicopter to Sacred Heart Medical Center at Riverbend in Springfield. Police say she died from injuries sustained in the crash.
OSP troopers are continuing to investigate the crash.
